National Standard 5

New Policy for Patient Identification and Procedure Matching (ratified 23/10/2013) 
 

To correctly identify a patient at BHS the following 3 key identifiers must be used: 

BHS Approved Patient Identifiers

Inpatients

  • Patient name (family and given names)
  • Date of birth
  • Medical record number (UR number)

Ambulatory Services

  • Patient name (family and given names)
  • Date of birth
  • Address
     

Incorrect identification can have drastic effects

  • Procedure performed on wrong patient
  • Wrong body part operated on
  • Wrong medications given leading to adverse events for the patient

All these events could lead to an increased length of stay and potentially causing serious harm to the patient which could lead to death
